- Physical examinations – new patients and annual check-up
- Sick visits (for Bergen Volunteer Medical Initiative patients only)
- Blood work
- Urinalysis (testing your urine)
- Immunizations (vaccination)
- Nutrition coaching
- CounselingReferrals to outside specialists
- Women’s Health Package for all women
- Diabetes Care and Education Program
- Prevent Diabetes Program (PDP)
- Chronic Disease Management Program to assist patients in managing
- High Cholesterol
- High Blood Pressure
- Obesity and Overweight
Patient eligibility
- Adults 18 years or older living in Bergen County with no health insurance
- Someone in the household must be working
- Earning no more than 250% of the Federal Poverty Level
- Do not qualify for Medicare, Medicaid, or subsidized health care
