ModivCare
What is Modivcare?
- ModivCare is the transportation vendor for people enrolled in Medicaid or NJ FamilyCare. It is used for non-emergency transportation for medical appointments. Members may call them at 1-866-527-9933 (TTY 1-866-288-3133) to set up a transportation appointment. You can also make your reservation online at modivcare.com. Call by noon at least 2 days before you need the ride.
- Don’t forget to schedule your return ride home; but if you did not schedule a ride, you can do so once your appointment is over. You or your doctor’s office can call the "Ride Assist" phone number at 1-866-527-9934 (TTY 1-866-288-3133) and ask someone to pick you up. The transportation provider should pick you up within 60 minutes from the time you call them.
Eligibility
- NJ Family Care or Medicaid holder
- Physical Conditions:
- For those who can walk without another person’s help
- For those who need assistance walking or are in a wheelchair
- For those who need a stretcher (not an emergency)
- Applicable appointments
- One time appointment
- Recurring appointments - 3 days or more per week (i.e. dialysis treatments)
- Be prepared to provide the following information:
- NJ Family Care/Medicaid ID number
- Your pick up address and zip code
- Name, phone number, and email address of your medical provider
- Appointment time and date
- Special transportation needs such as assistance walking or if you are in a wheelchair, as well as of you need a stretcher (non-emergency)
How to Make a Reservation
- Call at least 2 business days before your appointment
- Call 1866-527-9933 (they will decide pick up time)
- Click #1 for English, #2 for Spanish, #3 for Russian
- For other languages, you can request a translator and give information about the appointment. The phone call system is automated. However, you can request to speak with a representative who will connect you with a language line interpreter for scheduling rides.
- Follow the prompts: filling out phone number, birthday, zip code, and the last 4 digits of your NJ Family Care/Medicaid member ID
- You will then be transferred to a representative. There you can request a translator and give information about the appointment.
- For the return trip, request for “Will-Call” then call when your medical appointment has finished. Modivcare will take up to 60 minutes to arrive with transport.
- Trips that are over the 20 mile cap will be denied in the system and reviewed through the Closest Provider Certification Process which may take up to 10 to 14 business days.

- Only the facility can request ongoing standing orders to the program.
- Call 1866-527-9933 to receive “Standing order form” then send the filled out form to FAX # 877-457-3316. Only the facility can request ongoing standing orders to the program.
- If the ride is late for more than 15 minutes, or you would like to file a complaint, call 1-866-527-9934
